(Main)Health informatics - System of concepts to support continuity of care (ISO 13940:2026)
General Information
- Abstract
This document defines the requirements for a system of concepts for different aspects of the provision of care encompassing social care as well as clinical care. The focus of this document is continuity of care.
This document defines requirements for a system of concept definitions needed to describe health and care businesses. Concept systems conforming to this document can be used to support the development of:
logical reference models within the information viewpoint as a common basis for semantic interoperability on international, national or local levels;
information systems;
information for specified types of care processes.
This document does not specify how to perform specific care processes. This document does not cover research processes in the context of social and clinical care, welfare and educational processes.

Overview
SIST EN ISO 13940:2026 - Health informatics - System of concepts to support continuity of care - establishes a standardized framework for defining concept systems essential to supporting continuity of care. This international standard, developed by SIST, addresses both social and clinical care, outlining requirements for concept definitions that describe health and care business processes.
The standard enables organizations and information systems to achieve greater semantic interoperability by providing a common conceptual foundation. Its primary value lies in facilitating consistent communication and data exchange across diverse healthcare environments, supporting delivery of coordinated, patient-centered care.
Key Topics
Continuity of Care Concept System
Addresses the definition and structure of concepts related to ongoing care provision, ensuring clinical and social care are integrated for better health outcomes.Scope of Concepts
Covers fundamental elements including:- General health terms
- Types of health and social needs
- Care actors and organizations
- Activities and processes involved in delivering care
- Relevant timeframes and events (e.g., appointments, episodes of care)
Semantic Interoperability
Provides requirements for information systems to build or adopt logical reference models that enable accurate, standardized data sharing at international, national, or local levels.Care Business Processes
Outlines how concept systems contribute to:- Specifying and tracking care workflows
- Identifying roles and responsibilities
- Managing episodes and transitions in care
Applications
SIST EN ISO 13940:2026 supports a wide range of healthcare and IT scenarios, including:
Electronic Health Record (EHR) Development
Ensures EHR solutions can reliably interpret and transfer data related to clinical and social care by standardizing core concepts.Care Coordination and Case Management Systems
Facilitates seamless transitions for patients across care settings, improving communication among clinicians, social workers, and care organizations.Health Information Exchange (HIE)
Enhances data compatibility between institutions, supporting population health initiatives and shared care models.National and Regional Health IT Frameworks
Supports policy makers and health informatics architects in defining interoperable health IT architectures that enable continuity of care.Research and Population Health Analysis
Provides a standardized reference for analyzing care processes and health outcomes, even though the standard itself does not specify research processes.
The standard is applicable to:
- Healthcare providers
- IT vendors
- System integrators
- Health authorities
- Standards developers
